POLICIES & PROCEDURES- THERAPY SERVICES AND CLIENT RIGHTS & RESPONSIBILITIES
Absences and Cancellations:
At least twenty-four (24) hours advance notice is required for all cancellations or rescheduling needs. Unless deemed an emergency, you will be charged a $35 fee for all notices under 24-hours. The full fee will be charged for sessions missed without such notification.
Terms of Agreement:
This Agreement will become effective when the Acknowledgement and Consent Form is signed by both parties. Maestro Music Therapy may terminate services with 30-days notice. All attempts will be made to give you the names of other qualified professionals. You have the right to terminate therapy at any time. Maestro Music Therapy shall be entitled to full payment for services performed prior to the effective date of termination.
Rights and Responsibilities:
As a client of Maestro Music Therapy I understand that I have certain rights and responsibilities that will help ensure a successful therapeutic experience.
You Have the Right To:
Refuse and/or terminate treatment at any time. The only requirement is that you pay for any sessions you have already received.
A safe treatment setting, free from sexual, physical, and emotional abuse.
Ask for and receive information about your therapist’s qualifications (e.g. certifications, education, training, experience, membership in professional groups, special areas of practice, and limits on practice).
Have written information, before entering therapy, about fees, confidentiality, method of payment, and cancellation policies. This is known as informed consent.
Request a second opinion.
File a complaint with the government or your music therapist’s professional association if you believe that you have been treated unethically.
Refuse to answer any questions or give any information you choose not to answer or give.
Share with your music therapist aspects of our session that you believe are helpful for you and which aspects are not.
Keep what you tell us private. Generally, information you reveal to your music therapist during treatment will be kept strictly confidential (understanding that any pertinent information relative to your care will be documented in a Maestro Music Therapy progress note record) and will not be revealed without your written authorization. There are some situations in which your music therapist may be required by law to reveal things disclosed in music therapy, even without your permission. These include:
If you seriously threaten to harm another person, your music therapist must warn that person and the authorities.
If a court orders your music therapist to testify about you.
If your music therapist has good reason to believe you are abusing, neglecting, or exploiting a child or vulnerable adult or if you give information about someone else who is doing this, your music therapist must notify Child Protective Services or Adult Protective Services.
If your music therapist believes you are in imminent danger of harming yourself, they may call the police, if all other options are exhausted.
If a medical emergency requires your music therapist to disclose information.
If you enter into litigation with Maestro Music Therapy
If your music therapist believes you are intentionally spreading communicable diseases.
Your Responsibilities as a Client/Parent/Legal Guardian:
You are responsible for actively participating in our agreed upon treatment and making a commitment to your success, which includes honestly addressing and sharing your thoughts and feelings, even if it is uncomfortable or difficult, following through with homework, actively participating in the development of your goals, making sure your goals are understood, and being on time for your sessions.
You are responsible for keeping predetermined appointments and giving Maestro Music Therapy at least twenty-four (24) hours notice of any cancellations or rescheduling needs. Otherwise, unless deemed an emergency, you will be charged a $35 fee. The full fee will be charged for sessions cancelled without such notification.
You, not your insurance company or any other person or company, are responsible for paying the fees we agree upon.
You are responsible for knowing how to contact your music therapist or other resources in case of any emergency.
You are responsible for maintaining confidentiality pertaining to group therapy, when applicable.
You are responsible for informing Maestro Music Therapy of any address or phone number changes that arise.
Scheduling of sessions:
Scheduling will be worked out between you and your music therapist. Sessions will occur in the Maestro Music Therapy studio, client’s home, via Telehealth platform, or some other pre-determined location.
Payment:
The rates for therapy sessions are as follows:
-Assessment session (one time fee, not time limited. Includes development of therapy goals and treatment plan, and consultation with caregivers.) = $100.00
(please ask your therapist if this fee applies to you)
-30-minute therapy session = $65.00
-45-minute therapy session = $80.00
-60-minute therapy session = $90.00
We accept check, cash or credit card through Venmo or PayPal. You will be emailed an invoice at the end of the month for services rendered that month (e.g. an invoice for September sessions will be mailed at the end of September or beginning of October). Payment is accepted online and is due in full within 30 days. Instructions for how to pay using your credit card or online checking will be included in the email invoice. Unpaid balances over thirty days will be subject to a late payment fee of $20 a month.
